Our family medicine doctors, case managers and physical therapists conduct home visits to monitor Your loved one’s historical health and help keep them healthy.

Your loved one’s historical health information and ongoing treatment plan are carefully curated and entered into an Electronic Medical Record (EMR), following visits to consultant clinics, as well as after emergency room visits such as hospitalization, self-grooming, eating, and 24/7 access to medical care.

The EMR dashboard is accessible via a secure app or website. All routine, urgent or emergency care communications with our team are through the Silver Care app.

  • a schedule for selecting your video call/home visits
  • a pre-visit questionnaire link to be filled by you Our team will coordinate a pre-visit video call within 2 working days, and a home visit within 5 working days after enrollment.

WHAT TO EXPECT DURING YOUR HOME VISITS

 

FAMILY MEDICINE DOCTOR

  • The Family Medicine doctor will conduct a detailed baseline assessment at home, with monthly follow-up thereafter.
  • Each 30 min follow-up visit will include a review of symptoms and wll being.

 

PHYSICAL MOBILITY AND WELLNESS MANAGER

  • Physical Mobility Manager will visit thrice a week for 45 min sessions
  • Each session will include basic exercises to improve mobility and muscle strength
  • As a wellness advocate he/she will assess mental health, review medications and document outcomes of ER or scheduled home visits.
