CONCIERGE HEALTHCARE FOR YOUR LOVED ONES
Elders and senior citizens require care in their old years as they begin experiencing health issues or at least face deteriorating motor skills. Many people who care about their elderly are unfortunately unable to care for them personally due to long-distances or sometimes a hectic schedule. Silver Care identified the problem and created an innovative solution for it. Instead of unnecessary shuttling of elderly between different care facilities, our in-home elderly care service provides personalized, preventative, physician-guided primary care experience in the comfort of your home.
HOME VISITS
ONCE A MONTH
The Family Medicine Doctor conducts a thorough medical examination and reviews ongoing treatment plans.
THREE TIMES A WEEK
The Physical Mobility and Wellness Manager conducts mobility exercises to help your loved one maintain or gain as much independence as they safely can. As a wellness advocate he/she will systematically assess physical mobility and mental health. Additionally they will capture new or revised prescriptions and enter requests or results for laboratory tests and procedures.
CONCIERGE SERVICE
Through the Silver Care app, you can request bookings for outpatient clinic visits. Your loved one will receive our concierge accompaniment service at select private hospitals and emergency rooms in the city.
HEALTH DASHBOARD
Our professionals will update your loved one’s electronic medical records following each interaction with Silver Care services, including after visits to outpatient clinics or emergency rooms.
WHAT TO EXPECT DURING YOUR HOME VISITS
FAMILY MEDICINE DOCTOR
- The Family Medicine doctor will conduct a detailed baseline assessment at home, with monthly follow-up thereafter.
- Each 30 min follow-up visit will include a review of symptoms and wll being.
PHYSICAL MOBILITY AND WELLNESS MANAGER
- Physical Mobility Manager will visit thrice a week for 45 min sessions
- Each session will include basic exercises to improve mobility and muscle strength
- As a wellness advocate he/she will assess mental health, review medications and document outcomes of ER or scheduled home visits.
HOME VISIT CHECKLIST
Well lit room with a bed for physical examination
Comfortable loose clothing that allows for examination
Collection of previous and Current labs, medical records, prescriptions and medications
